Health Coaching
Ongoing support for turning health goals into sustainable action. Health coaching has its own membership and transparent pricing.
Psychiatric Care & Therapy
Psychiatric evaluation, medication management, and therapy provided by our PMHNP and billed to insurance. Your costs depend on your individual plan.
Annual Healthspan Assessment
A separately billed, cash-based assessment of strength, mobility, balance, and cardiovascular fitness with our independent physical therapy partner.
Before You Join...
Insurance and outside costs: Phoenix Rising does not bill insurance for Primary Care membership services. Laboratory testing, imaging, medications, specialist care, and services from outside healthcare professionals are not included in the membership. Depending on the provider and service, these costs may be billed to insurance or paid directly by you.
HSA/FSA funds: Your membership may be eligible for payment or reimbursement through an HSA or FSA. Beginning in 2026, federal rules allow HSA funds to be used for qualifying direct primary care membership fees. Eligibility varies, so please confirm coverage with your individual plan administrator.
Team-based services: Health coaching has its own separate membership and pricing. Psychiatric care and therapy provided by our PMHNP are billed to insurance and are subject to your plan’s coverage, deductible, copay, and coinsurance.
Annual Healthspan Assessment: This assessment is completed during the new-patient process and annually thereafter through our independent, cash-based physical therapy partner in Turner, Oregon. It requires a separate appointment and fee and is not included in your Primary Care membership.
Who we see: The Midlife Health Clinic serves adults age 18 and older and does not provide pediatric primary care.
