Our Annual Healthspan Assessment evaluates the strength, mobility, balance, and cardiovascular fitness that help you remain active, capable, and independent as you age.
The assessment is completed during the new-patient process and annually thereafter.
It requires a separate appointment with our independent physical therapy partner and is not included in the Primary Care membership; a separate fee applies.
Measure What Matters
A traditional annual exam can identify disease and important health risks. It does not directly measure how your body performs.

A Personal Roadmap
The assessment is only one part of the picture.
Your primary care provider reviews the results alongside your medical history, laboratory findings, cardiovascular and metabolic risk, and—when appropriate—DEXA or other clinical testing.
The Phoenix Healthspan Calculator and your personalized Phoenix Roadmap help bring this information together with your emotional, mental, spiritual, and overall well-being, priorities, and goals.
